Dato' Dr. Sahabudin Raja Mohamed is a senior urologist with over 38 years of experience, 22 years of experience in robotic surgery and minimally invasive urological surgery. He pioneered robotic surgery in Malaysia in 2003 and is a leading expert in the treatment of prostate disease, urinary stones, and reconstructive urology. Ureteroscopic Extraction of Stones

This treatment is used to remove stones from the ureter or kidney, especially when they are causing pain, blockage, or infection. It is ideal for stones that are too large to pass on their own or have become stuck in the urinary tract.

Percutaneous Nephrolithotomy

PCNL is used to remove large or complex kidney stones that cannot be treated with simpler procedures. It is especially helpful for stones that fill a large part of the kidney or are resistant to other treatments.

Retrograde Intrarenal Surgery

RIRS is used to treat kidney stones that are small to moderate in size, especially those in areas that are difficult to reach. It is often recommended when other methods, like shockwave therapy, aren’t suitable.

Extracorporeal Shock Wave Lithotripsy

ESWL is a non-invasive treatment for kidney or ureteric stones. It is typically used for smaller stones that are located in the upper part of the urinary system.

Robotic Assisted Radical Prostatectomy

This surgery is done to treat prostate cancer that is confined to the prostate gland. It involves removing the prostate before the cancer has a chance to spread.

Transurethral Resection of Prostate

TURP is used to relieve urinary symptoms caused by benign prostatic hyperplasia (BPH), a non-cancerous enlargement of the prostate. Common issues include slow urine flow, frequent urination, and incomplete bladder emptying.

Transurethral Resection of Bladder Tumour

TURBT is performed to remove bladder tumors and confirm a diagnosis of bladder cancer. It is the first and most important step in managing early-stage bladder cancer.

Laser prostatectomy is used to treat an enlarged prostate (BPH) that causes difficulty urinating. It is often preferred for patients at higher risk of bleeding or who are on blood-thinning medications.

Nephrectomy
Treatments

A nephrectomy is the surgical removal of a kidney. It is most commonly performed to treat kidney cancer, severe infections, or a non-functioning kidney. It is also done for healthy kidney donation in transplant cases.

Pyeloplasty – Open and Robotic

Pyeloplasty is a surgical procedure to correct a blockage at the point where the kidney connects to the ureter (the ureteropelvic junction). This blockage may be present from birth or develop due to scarring or stones.

Laparoscopic urologic surgery is used to treat a wide range of conditions, including kidney tumors, adrenal growths, ureteral blockages, and other urinary system problems. It offers a minimally invasive alternative to open surgery.

Open Radical Prostatectomy

Open radical prostatectomy is a surgery to remove the entire prostate gland in men with prostate cancer. It is often chosen when robotic or laparoscopic surgery is not suitable or available.

Cystectomy with Ileal Conduit/Continent Diversion

Cystectomy is the surgical removal of the bladder, usually done to treat muscle-invasive bladder cancer. After removing the bladder, a new way to store and pass urine must be created using a diversion technique.

These procedures help repair, maintain, or improve parts of the urinary system affected by blockages, injuries, birth defects, or previous surgeries. They also include minor interventions that make daily life easier or prepare for other treatments.

Ureteral
Reimplantation

Ureteral reimplantation is a procedure done to correct problems with the way the ureter connects to the bladder. It is often used to treat vesicoureteral reflux (VUR), where urine flows backward from the bladder into the ureters, or to repair damage from injury or previous surgery.

Penile Reconstruction Surgery

Penile reconstruction surgery is performed to rebuild or restore the penis in patients who have congenital abnormalities, trauma, cancer, or as part of gender-affirming surgery. It aims to restore both function and appearance.

Robotic Vesicovaginal Fistula (VVF) Repair

Robotic VVF repair is used to correct an abnormal connection (fistula) between the bladder and vagina, which causes continuous urinary leakage. This condition can result from childbirth complications, surgery, radiation, or injury.

Robotic Cystoprostatectomy

Robotic cystoprostatectomy is the surgical removal of the bladder and prostate, usually done to treat invasive bladder cancer in men. It’s often followed by urinary diversion, where a new way to store and pass urine is created.

Biopsy

A prostate biopsy is a procedure done to check for prostate cancer or other abnormalities. It is usually recommended if a blood test (PSA) is elevated or if a doctor feels a lump or hardness during a rectal exam. It helps confirm whether prostate cancer is present and how aggressive it may be.

Men’s Health
(e.g., Erectile Dysfunction)

Erectile dysfunction is the difficulty in getting or maintaining an erection firm enough for sex. It can affect confidence and relationships but is highly treatable.

Treatments

Lifestyle changes, oral medications (such as phosphodiesterase-5 inhibitors), vacuum erection devices, penile injections, or penile implant surgery.

Urinary Tract, Kidney, and Bladder Stones

Stones are hard mineral deposits that can form in the kidneys, ureters, or bladder. They can cause pain, infection, or block urine flow.

Treatments

Observation for small stones, extracorporeal shock wave lithotripsy (to break stones using sound waves), ureteroscopy or retrograde intrarenal surgery (to remove stones via scope), or percutaneous nephrolithotomy (keyhole surgery to remove large kidney stones).

Kidney, Bladder, Prostate, and Testicular Cancer

These cancers affect the urinary and reproductive organs and may cause blood in urine, pain, or lumps.

Treatments

Surgery (such as partial or complete removal of kidney, bladder, prostate, or testicle), chemotherapy, radiation therapy, and immunotherapy.

Male and Female Urinary Incontinence

Incontinence is the involuntary leakage of urine. It can result from weak pelvic muscles, nerve damage, or prostate issues.

Treatments

Pelvic floor muscle training, bladder training, medications, urethral bulking injections, catheter use, sling procedures, or robotic surgery to repair urinary fistulas.

Sexually Transmitted Diseases (STDs)

These are infections passed through sexual contact and can affect the urinary tract, genitals, or entire body.

Treatments

Treatments: Antibiotics for bacterial infections (such as gonorrhea or chlamydia), antiviral medications (for herpes or HIV), and safe sex education.

Male Infertility

This refers to a man’s difficulty in fathering a child, often due to low sperm count, poor sperm quality, or blockages.

Treatments

Surgery to correct blockages (such as varicocelectomy or vasectomy reversal), hormone therapy, or assisted reproductive techniques such as in vitro fertilization (IVF) or intracytoplasmic sperm injection (ICSI).

Prostate Cancer

This is a common cancer in men that can affect urination and sexual function. Early detection greatly improves outcomes.

Treatments

Active surveillance (for early cases), surgery to remove the prostate (prostatectomy), radiation therapy, hormone therapy, or chemotherapy.

Kidney, Bladder, and Urethral Injuries

These injuries can occur due to accidents, trauma, or surgery, leading to bleeding, leakage, or blockage.

Treatments

Bladder catheterization, surgical repair, reconstructive procedures (such as urethroplasty), and in severe cases, removal of damaged tissue.

Urinary Tract Infection (UTI)

UTIs are infections of the bladder, urethra, or kidneys, often causing burning during urination, urgency, or fever.

Treatments

Antibiotics, increased fluid intake, pain relief medication, and further tests to find any underlying cause such as stones or structural abnormalities.

Benign Prostatic Hyperplasia (BPH)

This is a non-cancerous enlargement of the prostate gland that causes urinary symptoms like slow flow or frequent urination.

Treatments

Medications to relax or shrink the prostate, minimally invasive procedures (such as transurethral resection of the prostate or laser surgery), or open/robotic prostate surgery for larger glands. Rezum for smaller prostates

Varicocele

Varicoceles are enlarged veins in the scrotum, similar to varicose veins, which can cause discomfort or infertility.

Treatments

Surgical ligation (varicocelectomy) or minimally invasive embolization to block abnormal blood flow.

Hydrocele

A hydrocele is a fluid-filled swelling around the testicle, often painless but sometimes uncomfortable.

Treatments

Surgical drainage or removal (hydrocelectomy), especially if the swelling is large or persistent.

Urinary System Blockages or Birth Defects

These are structural issues present at birth or acquired later, causing difficulty passing urine or recurrent infections.

Examples

Ureteropelvic junction obstruction, posterior urethral valves, urethral strictures, or urinary fistulas.

Treatments

Reconstructive surgery (such as pyeloplasty or ureteral reimplantation), urethroplasty, robotic repair, or use of stents and dilators.

Prostatitis

Prostatitis is inflammation of the prostate gland, which sits below the bladder in men. It can cause pelvic pain, trouble or frequent urination, and sometimes fever or fatigue. It may occur suddenly (acute) or develop gradually and last for months (chronic). Causes include bacterial infection or other non-infectious factors, and it may be acute or chronic.

Treatments

Bacterial prostatitis is treated with antibiotics, while other cases may use anti-inflammatory medicine, warm baths, and lifestyle changes like drinking more water and limiting caffeine.
